Merivale Medical Practice is a member of the Partnership Health PHO (Primary Health Organisation), which provides funding and subsidised care and special programs for those who are enrolled patients. The aim of this funding is to benefit and meet the health care needs of the local community.

You can call into Merivale Medical Practice and ask for an enrolment form our reception team or click on the Enrolment form PDF below to bring up the form for printing off at home.
Fill it out and return it to us by hand or post or email office@merivalemedical.co.nz
